How To Access Services

Access

The Government funds Disability Support Services (DSS) through the Ministry of Health and the District Health Board. The Supportlinks Needs Assessment Service Coordination (NASC) service is one type of disability support service. To access other Ministry of Health and District Health Board funded disability support services such as personal care, household management and carer support, you must first have a needs assessment with Supportlinks.

Referral

Anyone can make a referral to Supportlinks, you can refer yourself. Some people prefer to talk to their family, a friend, doctor, social worker or other health professional and ask them to complete the referral form. Your consent is required before another person can make the referral.

When my needs change

Review/Reassessment

Each year we will make contact with you to review your support plan and to monitor how the supports have been working.

Change in need

You can contact Supportlinks at any time your needs change. We can arrange a review of your support plan or a reassessment of your need.

Your Rights

You will receive information about the when we contact you about your assessment. Your rights will be explained to you by the Needs Assessor. Health & Disabilty Commissioner’s Code of Rights when we contact you about your assessment. Your rights will be explained to you by the Needs Assessor.
