Procedure for housing assessors (OT)

Please use this procedure when requesting minor ACC housing modification services from Enable New Zealand. 

This procedure starts when you have completed an onsite assessment and identified a need for minor modifications.

What to do

  1. 1

    Get sketches and quote from builder, if required

    To book a builder for a site meeting, contact BGIS. This is our design and build service provider.

    How to contact BGIS

    You’ll need to provide this information: 

    • Date of the appointment. At least 36 hours’ notice is preferred. 

    • Client’s name, address and ACC claim number.

    • Any risks or hazards with the site visit.

    • A summary of the injury related need and the solutions(s) proposed.

    The builder must provide a sketch and a quote within five working days.

    Order MRES housing equipment, if required

    To request the equipment portion of a housing modification: 

    • Submit an order for housing equipment in MRES.  

    • Attach the quote and completed assessment report (ACC263) to the MRES order.  

    • Once a purchase order is generated by the ACC Equipment team, MRES will automatically generate, and send a copy of the ACC96 form and quote to the case manager.

    Use form ACC1563 (embedded in ACC263) to get the property owner’s consent. Enable New Zealand can assist with obtaining the property owner consent, if required.

    Email these documents to our ACC Housing team:

    • ACC263 Assessment report - minor modifications

    • ACC1563 Housing modifications consent (embedded in ACC263)

    • ACC1564 Housing modification responsibilities form (embedded in ACC263)

    • Builder’s sketch and quote.

    Clinical Review

    Our Clinical Housing Team will review the referral to ensure the proposed modifications meet the client’s injury‑related needs. They will request additional information if required.

    Approval to proceed to construction

    We’ll request ACC approval if required, after which: 

    • We will instruct the builder to go ahead with the modifications. 

    • We will notify the Assessor and ACC of build dates and estimated completion once BGIS has confirmed with the client. 

  7. 7

    Inspect completed work

    We'll let you know when the work is completed.

    • You, the ACC client, and the builder need to complete form ACC1565 Certificate of satisfactory completion.

    • We notify ACC that the modifications have been completed.

    Complete our survey

    Please let us know if the modification provided the outcome you expected for your client.
