Assessor information

On this page you'll find resources and further information for assessors involved in Mobility for Life

Assessors are the clinicians who work with people to understand their mobility needs and decide whether a power wheelchair is the right option. They play a key role in identifying and supporting people who may benefit from taking part in the project.

Assessors will:

  • Identify and assess people who meet the eligibility criteria.

  • Use their clinical judgement to determine suitability for a power wheelchair.

  • Complete and submit required referral and assessment documentation.

  • Continue to support the person with fitting, training, and follow‑up as usual.

Participation in the project does not change clinical autonomy or decision‑making responsibilities.

How to submit an eligible person

If you think someone you're working with may be a good fit, the first step is to check they meet the eligibility criteria and have a kōrero with them about the project.

If they're interested, you can put them forward by submitting a referral and outcome summary, a wheelchair specfication, and a WATCh outcome assessment through ProWorkflow.

WATCh and WATCh-Ad downloads coming soon.

You don't immediately need to include a power wheelchair quote at referral - that's requested later.

From there, the Mobility for Life Administrator checks the paperwork and passes the referral to the Clinical Lead for review. The Clinical Lead will either approve it, ask for more information, arrange a joint telehealth session with you and the person, or decline it if the criteria aren't met. You'll be kept in the loop through ProWorkflow.
