-Update from the Enable New Zealand Hearing team -

There is a change to the way you can access the privacy and consent form, when applying for the hearing aid funding application.  This form is available for all customers.

Previously, customers have been asked to sign it during their visit with the audiologist, whether they have new or replacement hearing aids or are trialling hearing aids.

The change, to ensure easy access to the form, is that the Assessor can download and print off the form and complete when the application is made.  We have now separated the consent part from the funding application form, making it available directly on our Enable website.

This form can be printed off at any time by finding it in our documents and forms library, the link below or in the ESH App.  

Please note:
The form will still be available at either the assessment or fitting appointments, to be signed by the caregiver. This change allows us to take trials into account and to support the applicant in some cases where the assessment and fitting takes place at the same appointment. It also will support caregivers where they might need more time to consider hearing aids for their child, as we recognise sometimes the assessment appointment is the first time they are made aware their child needs hearing aids.
